By checking this box, you agree to a twelve month membership agreement billed in one payment in full for $356 which is a savings of 20% over monthly payments of $37 which wouldl be $444 per year.  After the twelve months is up, this plan will automatically renew unless you give 30 days notice in writing.  By checking this box, you understand that this is a contractual agreement for 12 months.
